Wellness Program - Complete by June 30, 2025
The University partners with Personify Health to deliver the UA Wellness Program. The program runs each year alongside our plan year - July 1 - June 30. The current plan year is set to end June 30. Mark your calendars to complete the program and earn your rebate!
To earn the rebate:
- Required - Health Risk Assessment (1 point)
- Required - Biometric Screening (1 point)
- 6 additional points
Reminders - added 6/17/2025
- How to register for your account as an employee or a spouse
- How to login (be aware of the nuances with the SSO and spouse accounts!)
- The rebate details (be sure you meet all the qualifications in November!)
- Complete the program
- Be enrolled on a UA Choice Medical Plan at the time the rebate is paid out
- Be a current employee at the time the rebate is paid out
